This book is designed to offer the reader first-rate guidance on shock management in the real world. Comprehensive, evidence-based, and up-to-date instruction is provided on optimal care of patients with different types of shock - septic, hemorrhagic, cardiogenic, anaphylactic, and obstructive - at all stages from initial response through to ICU admission. As well as management, the coverage encompasses pathophysiology, clinical presentation, diagnosis, and emerging trends. A further key feature is the use of a scenario-based approach to present a series of cases based on real-life experiences. Here, a narrative style and Q&A form are employed to vividly convey scenarios that may be encountered in clinical practice and to elucidate decision making in complex circumstances. When readers experience difficulty in answering the questions, the earlier sections can be consulted to identify the correct response. This book will be of great value for all health care professionals. In particular, it will be very helpful for novice or inexperienced practitioners in emergency medicine, critical care medicine, and traumatology.

The aim of this book is to identify and shed new light on the main surgical practices involved in acute care and trauma surgery. Adopting an evidence-based approach, a multidisciplinary team of surgeons and intensivists illustrate basic and advanced operative techniques, accompanied by a comprehensive and updated reference collection. Emerging surgical procedures in Acute Care Surgery are also explored step by step with the help of ample illustrations. Further, the book offers an overview of treatment options for acute organ failure, with a focus on peculiar clinical situations that call for special treatment, including: extracorporeal membrane oxygenation (ECMO), circulatory support and urgent liver transplantation. In order to identify the key determinants in decision-making and therapeutic support that can help avoid critical errors, authors not only define and discuss specific surgical techniques, but also present major issues and controversies related to it. In essence, the book can be considered a "What to do guide" that offers easy access to both basic and advanced lifesaving surgical procedures, due in part to text boxes highlighting "Tips, Tricks and Pitfalls" as well as advice on "Complications Management". Many chapters are enriched by providing clinical case scenarios, an important educational resource including self-assessment questionnaires.
This Open access book offers updated and revised information on vessel health and preservation (VHP), a model concept first published in poster form in 2008 and in JVA in 2012, which has received a great deal of attention, especially in the US, UK and Australia. The book presents a model and a new way of thinking applied to vascular access and administration of intravenous treatment, and shows how establishing and maintaining a route of access to the bloodstream is essential for patients in acute care today. Until now, little thought has been given to an intentional process to guide selection, insertion and management of vascular access devices (VADs) and by default actions are based on crisis management when a quickly selected VAD fails. The book details how VHP establishes a framework or pathway model for each step of the patient experience, intentionally guiding, improving and eliminating risk when possible. The evidence points to the fact that reducing fragmentation, establishing a pathway, and teaching the process to all stakeholders reduces complications with intravenous therapy, improves efficiency and diminishes cost. As such this book appeals to bedside nurses, physicians and other health professionals.
Advances in intensive and critical care medicine occur rapidly. Innovation, training and research are the pillars that support a process deriving from basic science and multiprofessional/multidisciplinary interventions. The APICE 2010 Yearbook highlights several innovations for optimising prevention and management measures for the critically ill by integrating diagnostic procedures with pharmacological and technological options. Considering that cerebral, respiratory, cardiovascular, renal and metabolic dysfunctions occurring during serious illnesses or syndromes represent risk factors for patient survival, the final goal remains multifaceted: to improve standards and quality of care; to introduce the expanding concept of clinical governance and professionalism; to focus on the importance of ethical principles for establishing a process of patient-centered and evidence-based care. For these reasons, medical professionals must reach a consensus regarding the importance of continuing medical education.

This title will provide an up-to-date review of the emerging practice of neurology telemedicine. Increasingly, neurology, like other disciplines of medicine, is implementing the technological advances and discoveries of the science of healthcare delivery to improve not only access and efficiency but outcomes and cost as well. For patients in remote and underserved areas, having a neurological condition means lengthy and costly travel to obtain specialist evaluation. Telemedicine has the ability to deliver such care to a patient's local community. Neurology telemedicine for acute stroke has already demonstrated clinical efficacy and cost effectiveness by raising the numbers of thrombolysis-eligible patients who can be treated in a timely fashion. More than 85% of leading US neurology departments currently use or plan to implement telemedicine within the next year. The US military has a limited number of neurologists - a store-and-forward consultation system has enabled military neurologists to deliver far-forward battlefield care for service members deployed overseas. The chapters in this book will review the use of telemedicine for the evaluation and treatment of patients with many common neurological conditions and will provide a practical guide for neurologists seeking to incorporate telemedicine into their daily practices.
This text presents a basic guide of the principles and applications of ultrasound in the critical care setting. The text also addresses the basic and clinical uses of ultrasound, including clinical cases at the end of each of the 7 sections of the clinical subdivisions. The final chapters focus on the issues of training, certification, credentialing and billing. These discussions make the text unique, as literature regarding monetary and training issues for ultrasound in the ICU is sparse. Each chapter is written by experts in the field, and supplemented with illustrations and links to videos of actual ultrasound examinations in patients. Ultrasonography in the ICU: Practical Applications will be of great utility to critical care specialists in the disciplines of surgery, medicine, anesthesiology and emergency medicine. In addition this book can be used as a teaching tool for critical care fellows and residents interested in further training in ultrasound for the critically ill patient.
