In response to persistent donor organ shortages, organs from marginal donors, such as expanded criteria donors (ECD) and donation after cardiac death (DCD) donors, are now accepted and have been successfully transplanted, reducing the waiting times for transplantation. Especially in Japan, transplantation of DCD kidneys has a relatively long history because of the difficulty or lack of national consensus in accepting brain death, which has made it possible to accumulate considerable clinical experience. Thus, the current organ shortage has stimulated interest in the use of marginal donors for transplantation. On the other hand, however, it is known that these organs have a high rate of delayed graft function and a more complicated postoperative course. These drawbacks have created the greatest clinical challenge in transplantation to date because of the current shortage and limitations of donors using ECD and DCD. This book, prepared by distinguished authorities in their fields, is intended for clinicians and researchers. It highlights the use of marginal donors as a comparatively novel source of transplantation organs and provides a thorough overview of marginal donors from their historical origins to recent clinical applications, including the state-of-the-art science of organ/donor management, procurement, and preservation. A symposium, "Perioperative Management of Pacemaker Patients", was held in May 1990 at the Karl-Franzens-Universitat, Landeskrankenhaus Graz, Austria. The purpose of the symposium, organized by the Departments of Anesthesiology and Surgery, was to discuss the current status of permanent pacemaker technology, emerging developments in this field, legal issues, indications and methods for tempor- ary perioperative pacing, and the potential for pacemaker malfunction and adverse patient-pacemaker interactions in perioperative and other medical settings. It was hoped that participants would come to some consensus concerning recommendations for satisfactory perioperative management of pacemaker patients. This symposium was probably the fIrst occasion ever that brought together representatives of the pacemaker industry, implanting physicians, and physicians responsible for the management of pacemaker patients in perioperative and other hospital settings to discuss these important matters. Certainly, it was recognized by all who attended that there existed at the time little in the way of substantial knowledge of how to best manage pacemaker patients or patients with possible indications for temporary pacing in "unique" hospital settings especially with exposure to electroma- gnetic and other potential interference, but in the absence of professional persons with direct knowledge of pacemakers and related devices. Much new information was presented and useful ideas exchanged, so that three symposium participants were of the opinion that papers presented at this meeting should be published as a collective work.

This book provides a comprehensive review of ethical issues in clinical nephrology. With the advent of dialysis and kidney transplantation midway through the 20 Century, clinical nephrology was one of the first areas of medicine to deal with complex ethical issues such as rationing of health care and discontinuation of life-sustaining therapy. In the first section of the book this historical perspective is reviewed, followed by a consideration of legal issues. Specific ethical issues in nephrology are discussed in detail in the next section. These include problems in the allocation of chronic dialysis and in termination of that treatment. Also reviewed are issues in kidney transplantation, such as proposals for enhanced acquisition of kidneys, including a number of controversial proposals such as payment to donors and xenotransplantation; and equity in allocation of the supply of kidneys. Other chapters consider ethical issues in genetics; special problems in the care of children with kidney disease; and broad societal issues such as allocation of national resources for expensive therapies and economic issues in clinical practice. Ascites is the most frequent and hepatorenal syndrome the most lethal complication in liver cirrhosis. In recent years, major progress has been made regarding effective treatment of these complications, thus reducing mortality in patients. The first experiments with neural transplantation into the brain (for a review, see Bjorklund and Stenevi 1985) were reported at the end of the last century by Thompson (1890), who took cortical tissue from adult cats and implanted the tissue pieces into the neocortex of adult dogs. The neurons did not survive, but the transplanted mass did not disintegrate entirely. Successful graft- ing into the brain was reported later by Ranson (1909), who implanted spinal ganglia into the cerebral cortex of developing rats, and by Dunn (1917), who reported survival of implanted neonatal cortex into the cortex of newborn recipients. Another important step was made by Le Gros Clark (1940) who reported that, in the rabbit, embryonic (E) cortical tissue could be successfully grafted into the cortex of young recipients. Ex- periments by Wallace and Das (1982), and Bjorklund and Stenevi (1984), showing behavioral effects of grafts, subsequently stimu- lated a growing interest in neurotransplantation research. Intrac- erebral grafting is now considered a powerful tool for addressing fundamental questions about development, regenerative, or re- storative phenomena in the central nervous system (CNS) and is a potential therapy for neurodegenerative diseases.
