This document contains reports of 31 cases, investigated by the Health Service Ombudsman, completed between April and September 2003. Some are given as short summaries, whilst others are reported in more detail. The investigations are grouped under five recurring themes: poor complaint handling; failure to secure informed consent; poor nursing care; lack of liaison between services; and liaison between GPs and other professionals. They cover; hospital, primary care and ambulance trusts; as well as family health service practitioners and health authorities.
