This manual is designed to orient helpers to offer psychological first aid (PFA) to people following a serious crisis event. PFA involves humane, supportive and practical assistance for people who are distressed, in ways that respect their dignity, culture and abilities. The instructions and materials in this manual are for a half-day orientation (4 hours excluding breaks) to prepare helpers to support people recently affected by very stressful events. If you have more time available for the orientation, extra activities and slides are included in text boxes to deliver a full day orientation (five and a half hours excluding breaks). Where possible, one should organize full day orientations to allow for deeper learning.It is important to adapt the orientation and materials to the local culture, language and context, and to the likely kinds of crisis situations in which your participants would be helping. Adherence to therapies is a primary determinant of treatment success. Poor adherence attenuates optimum clinical benefits and therefore reduces the overall effectiveness of health systems. Medicines will not work if you do not take them. Medicines will not be effective if patients do not follow prescribed treatment - yet in developed countries only 50% of patients who suffer from chronic diseases adhere to treatment recommendations. In developing countries, when taken together with poor access to health care, lack of appropriate diagnosis, and limited access to medicines, poor adherence is threatening to render futile any effort to tackle chronic conditions such as diabetes, depression and HIV/AIDS. This report is based on an exhaustive review of the published literature on the definitions, measurements, epidemiology, economics, and interventions applied to nine chronic conditions and their risk factors. These are: asthma, cancer (palliative care), depression, diabetes, epilepsy, HIV/AIDS, hypertension, tobacco smoking, and tuberculosis. Intended for clinical practitioners, health managers and policy-makers, this report provides a concise summary of the consequences of poor adherence for health and economics. It also discusses the options available for improving adherence, and demonstrates the potential impact on desired health outcomes and health care budgets. It is hoped that this report will lead to new thinking on policy development, and action on adherence to long-term therapies.
This book evaluates the risks to human health and the environment posed by the use of Bacillus thuringiensis (Bt) as a microbial agent for pest control. Products containing various Bt subspecies are increasingly used worldwide to control the larvae of several insect pests that threaten major agricultural crops and forests. Bt products are also being used to control the insect vectors of malaria onchocerciasis and other diseases of major public health importance. The bacterium is also a key source of genes for transgenic expression to provide pest resistance in plants and microorganisms. The report opens with an overview of the biological properties of Bt and commercial Bt products. Particular attention is given to the mechanisms by which sporulation produces inclusion bodies containing insecticidal crystalline proteins which are selectively toxic for insect species in the orders Coleoptera Diptera and Lepidoptera. Tables show the current classification of 67 Bt subspecies and the large number of genes coding for the insecticidal crystalline proteins. A review of Bt metabolites found in commercial products concludes that they pose no hazards to humans or the environment. Chapter two reviews data elucidating the mechanisms by which Bt exerts its toxic action on susceptible insect larvae. Data on insect populations that are resistant to Bt are also briefly considered. Chapter three which focuses on the survival and activity of Bt in the environment compares habitats where Bt subspecies occur naturally with treated habitats. Particular attention is given to the ability of Bt to form endospores that are resistant to inactivation by heat and desiccation and that persist in the environment under adverse conditions. A chapter on commercial production describes methods of production and general patterns of use in agriculture and forestry and in large-scale programmes to control the vectors of malaria and onchocerciasis. The most extensive chapter evaluates the large number of studies conducted to assess the toxicity of various preparations containing insecticidal crystalline proteins spores and vegetative cells. Laboratory studies in a range of species have failed to demonstrate toxic or pathogenic effects. Field studies have likewise failed to demonstrate adverse effects on birds earthworms fish other aquatic vertebrates and non-target aquatic invertebrates. An evaluation of effects on humans draws on studies in volunteers case reports from occupationally-exposed workers and extensive data from countries where Bt products are added to drinking-water for mosquito control or used to treat rivers for blackfly control. On the basis of this review the report concludes that Bt products are unlikely to pose any hazard to humans or other vertebrates or to the great majority of non-target invertebrates provided the commercial product is free from non-Bt microorganisms and biologically active products other than the insecticidal crystalline proteins. The report further concludes that Bt products can be safely used for the control of insect pests of agricultural and horticultural crops and forests. These products are likewise judged safe for use in aquatic environments including drinking-water reservoirs for the control of mosquito black fly and nuisance insect larvae. The report stresses however that vegetative Bt has the potential to produce Bacillus cereus-like toxins whose significance as a possible cause of human gastrointestinal disease remains unknown. ...one of the most thorough published reviews of Bacillus thuringiensis ... provides an array of pertinent background material... -- Integrated Pest Management Network

"The International Classification of Diseases for Oncology" (ICD-O) has been used for nearly 25 years as the standard tool for coding diagnoses of neoplasms in tumor and cancer registrars and in pathology laboratories. ICD-O is a dual classification with coding systems for both topography and morphology. The topography code describes the site of origin of the neoplasm and uses the same 3-character and 4-character categories as in the neoplasm section of Chapter II, ICD-10. The morphology code describes the characteristics of the tumor itself, including its cell type and biologic activity.In preparing this revised edition, the editors have made a special effort to change as few terms as possible, to add new terms at empty spaces, and to avoid reuse of previously assigned codes. While all topography codes remain the same as in the previous edition, morphology codes have been thoroughly reviewed and, where necessary, revised to increase their diagnostic precision and prognostic value.

In October 2004 the Research Center for Gastroenterology and Liver Diseases of Shaheed Beheshti University hosted in Tehran an Iranian-American workshop on Food Safety and Surveillance Systems for Foodborne Diseases. The purposes of the workshop were to initiate contacts between Iranian and American specialists, exchange information about relevant activities in the two countries, and set the stage for future cooperation in the field. The participants also identified important aspects of food safety that should be addressed more intensively by both countries, including surveillance, research, international trade, and risk assessment. The framework for the workshop had been developed during a meeting of Iranian and American specialists in June 2003 in Les Treilles, France. More that 100 specialists participated in the workshop in their personal capacities, along with representatives of the World Health Organization and the Food and Agriculture Organization. These proceedings include a number of papers that were presented at the workshop together with summaries of discussions following presentation of the papers.
The monographs in this volume summarize the safety data on 11 pesticides that could leave residues in food commodities. These pesticides are aminopyralid, atrazine, azinphos methyl, lamba-cyhalothrin, difenoconazole, dimethomorph, flusilazole, procymidone, profenofos, pyrimethanil and zoxamide. The data summarized in the toxicological monographs served as the basis for the acceptable daily intakes and acute reference doses that were established by the Meeting.This volume and previous volumes of JMPR toxicological evaluations, many of which were published in the "FAO Plant Production and Protection Paper" series, contain information that is useful to companies that produce pesticides, government regulatory officers, industrial testing laboratories, toxicological laboratories and universities.
