Advocates of the perineal route have always aimed to impress us with the fact that in this method of operation the convalescence is shorter and the mortality smaller, especially the latter. Therefore their preference. Those advocating the suprapubic operation, as a reason for their preference, claim that this route of attacking the prostate offers the opportunity not only of the removal of the gland, but in addition we gain the advantage of ocular inspection of the bladder, the easy access in the removal of large stone from the viscus if present, reduction in the chances of possible injury to surrounding structures, and better urinary results. Many points regarding the comparative merits of the two operations have been intentionally eliminated for the reason that it has not been the writer's purpose to again discuss the so oftrepeated surgical topic, but to call attention to the report of H. Lilienthal and the results obtained by him in the two-step operation in the removal of the prostate. He has practiced the two-stage operation since 1896 in selected cases--when the patient appeared too weak to endure the operation at one sitting. For the past two years, however, he has performed all his prostatectomies by this method. He never lost a patient when the two-sittings operation has been performed. Unfortunately, he failed to mention the number of operations performed. He reports a portion (sixteen cases) of his two-stage prostate work where the average age of the patients was seventy-one years.
